Avalon Academy requires that all enrolled students follow the Texas Department of Health requirements for immunizations and provide a copy of their most current record,  or have a current signed affidavit on file for the exemption from specific immunizations.

As a licensed child care provider, Avalon may access Texas Department of Health’s ImmTrack system for confirmation of your child’s immunizations.

4 DPT’s – one of those doses being after the 4th birthday

3 Polio’s (OPV) – one dose being after the 4th birthday

2 Rubeola’s (M) – 1st on or after 1st birthday
(2nd at least 20 days after the first one)

1 Rubella (R) German Measles – on or after the 1st birthday
